REPOST: Student Immunization Forms …

The University has created a new immunization form. This form is for all new students enrolling at UTC in the fall 2009 semester.

It is very important that you bring your COMPLETED form to your orientation session. The main difference in the new form is that it REQUIRES the signature of the health care provider who gave you the vaccination.

my son will be a new student this fall,i have is records of his shots, that would that be ok, to use,
my question,is the doctor he has used has moved,but i do have the records of his shots, please let me know what i need to do….

@Robin – If your son graduated from a Tennessee High School then you are exempt from the Doctor’s signature on the form. You must still fill it out. You can also waive the Hepatitis B and the Meningitis vaccines. However, we certainly recommend that students get these, especially if they are living on campus. These shots are available at UT Family Practice and at the Health Department (which is located next to campus).

If he didn’t graduate from a Tennessee High School than we do need a doctor’s signature verifying vaccination. You can also get a doctor to run a titer for the disease.

i have no idea how to go about getting my immunization records. are they able to be transferred from my previous college?

@jonathan – If you graduated from a Tennessee High School since 1999 then you do not have to worry about getting those records. If you do need the records, then you can try to locate the doctor that gave you the shots. You can also get the shots again at a Health Department location (there is one right across from campus). You can also show that you have received the shots by getting a titer drawn by UT Family Practice (our student health center) or the Health Department.
