If you are enrolled in the Partnership PPO (Blue Cross or CIGNA), there are two things you and your eligible spouse must do by June 30, 2011:
- Complete a health questionnaire
- Complete a FREE health screening either:
a. At the University Center (see dates/times below)
(To schedule an appointment call 1-888-741-3390 or through an online account with APS) OR
b. Annual physical with your doctor
(He/she must complete the physician screening form and submit to the address on the form.)
All plan members can create an online account with APS Healthcare for the ParTNers for Health Wellness program. This online account will allow you to complete the health questionnaire and schedule a worksite health screening as required by the Partnership Promise. From the home page, simply click on “Complete Your Health Questionnaire” or “Set your FREE Health Screening” found in the upper right corner of the page. Then simply follow the onscreen instructions. Please note, members will need their Edison ID number, which is the same as the member ID number found on the Caremark prescription card. When asked to enter the Group Number, enter Partners for Health.
To schedule an appointment for APS screenings in the University Center, Chattanooga Room A, call 1-888-741-3390 or through your online account. Screenings will be held:
Thursday, January 27 from 8 am to noon
Friday, January 28 from 8 am to noon
Tuesday, February 1 from 8 am to noon
Wednesday, February 2 from 8 am to noon
Before attending the health screening, please note that your Edison ID is needed and can be found on your Caremark prescription card, so bring your prescription card with you to your health screening. For accurate results, fasting for 9 hours is required. This does not include medication, water, or black coffee. Screenings take approximately 10-15 minutes to complete.
It is not necessary to go to both a worksite health screening and primary care provider.
You cannot lose health insurance coverage because of your participation or your results. You must complete these requirements by June 30, 2011. If you don’t, you will be enrolled in the Standard PPO effective January 1, 2012.
