In compliance with a requirement issued by the UT Board of Trustees, all faculty, staff and students must receive flu vaccine this year. Exceptions to this requirement are permitted for those with medical and/or religious exemptions for receiving the vaccine.
Remember that getting flu vaccine early this year is especially important to help prevent influenza with the added presence of COVID-19.
Procedure for Complying with Vaccine Requirement
Verification of faculty and staff compliance with receiving the vaccine will be managed via self-disclosures within the My IRIS Web portal:
- Log in to My IRIS Web portal
- Choose Employee Self-Service
- Access your Personal Information
- Complete the Vaccination Verification form
The form allows you to confirm the date you received the flu vaccination or confirm which exemption category is applicable to you. Documentation is not required.
Use of the IRIS vaccine verification form requirement applies only to faculty and staff.
Guidance for students is provided by University Health Services.
If you are an employee who also is enrolled as a student at UTC, you are required to also complete the student vaccination verification procedure, which requires proof of immunization in addition to the employee IRIS form.

Vaccination Verification Form Instructions are available to assist you with learning how to access and complete the e-form.
A short Vaccine Verification Form training also is available on the K@TE learning management system. You can receive 0.5 hours of training credit by clicking through all parts of the training. Access the training at kate.tennessee.edu and enter “vaccine” in the search bar.

Free Flu Vaccination
Free flu vaccine is available to members of the state group insurance program.
Members may get a free flu shot vaccination by using:
- their Caremark cards at participating network pharmacies
or
- their health insurance cards (BlueCross BlueShield or Cigna) at their in-network doctor’s offices.
