This screening is by appointment and at NO COST when you bring your insurance information to your appointment. After your screening, have a light snack and enjoy our health education resources. For questions related to this process, call 425-2260.

Get Ready for the Screening – follow these steps 1, 2, and 3!
- Go to https://my.questforhealth.com and register or login. The registration key is SOT; your unique ID is your Edison ID which is found on your Caremark card. Download and print the Physicians Results Form. If you are unable to register online or need assistance with this process, call 1-855-623-9355.
- The night before your appointment, do not eat after midnight, but drink plenty of water. Take any regularly scheduled medications as prescribed by your healthcare provider.
- Bring the Physicians Results Form and State of Tennessee and Higher Education insurance card to your appointment. We will measure your height and weight, take your blood pressure and draw blood for labs.
Get Lab Results and Cash! When your lab results are back, UHS staff will complete your form and fax it to Quest Diagnostics at 1-844-560-5221.If any of the lab results are out of normal range, UHS will send you a secure message through the Medicat patient portal.
